ABSTRACT

Objectives: This was a single-subject study, aimed to demonstrate different vocal demand situations that are typical for primary school and teacher's vocal demand response under two acoustical conditions, with and without voice amplification, during five working days. Methods: The long-term voice dosimetry with Vocal Holter Med (PR.O. Voice Srl) was carried out on a 49-year-old female teacher with voice disorders during daily teaching activities. A sound field amplification system (SFAS) PentaClass Runa was installed in the classroom. Voice dosimetry was provided under two different acoustical conditions: without SFAS (2 days) and with SFAS (3 days). Results: Phonation time percentage, sound pressure level (SPL), SPL SD, fundamental frequency (F0), F0 SD, cycle, and distance doses were investigated in seven communication scenarios (lessons, group/individual classes, sports lessons in the gym and schoolyard, breaks, lunch breaks, and other activities). The median scores of all voice parameters differed significantly between different vocal demand contexts. The significant statistical difference in the vocal demand response was in the communication situations with and without SFAS. In addition, the number of children, reverberation time, and ambient air relative humidity impacted voice SPL and the cycle dose. Conclusions: Lessons, sports lessons held in the gym or schoolyard, breaks, and lunch breaks were considered as high vocal demand communication situations requiring higher voice intensity and fundamental frequency, higher phonation time percentage, cycle, and distance doses. Group/individual work and other teacher activities during the day, unrelated to direct work with students, were categorized as low vocal demand communication scenarios.

ABSTRACT

This paper focuses on the current state of the semiconductor industry at the global level as well as in India. With semiconductor chips emerging as the new gold, its demand is highest now than it was ever before. The demand for semiconductor chips in India is said to reach a gure of whopping US 110 billion dollars by 2030. In recent years, this industry has witnessed a crunch in its supply due to supply-side and demand-side constraints. This paper sheds light on the causes of these limits. Moreover, many countries are acquainted with this industry's scope in the future. India recognizes its dependence on countries like China for semiconductors. So, it aims to become a manufacturing hub for this industry and not just remain a fabless industry. However, India is encountering hurdles in this process. With this perception, this study has tried to explore India's initiatives in this direction. Keeping in mind India's vision of Aatmanirbhar Bharat, it seems like it's a long and rough road ahead to make the semiconductor industry in India, a self-reliant industry. This analytical study infers that overcoming these obstacles is going to become increasingly onerous.

ABSTRACT

Abstract: The phenomenon of oxygen debt (dO2) described several decades ago in the context of physical exercise has been incorporated into medicine, particularly during the hemodynamic changes produced by surgery and anesthesia in high-risk patients. dO2 is defined as the increase in the amount of oxygen consumed by the body immediately after physical exercise until O2 consumption returns to normal. In the perioperative period, an imbalance between oxygen supply (DO2) and demand (VO2) could generate dO2. The degree of tissue dO2 has been directly related to multiple organ failure and perioperative morbimortality. Despite advances in medicine, it is not yet possible to prevent or lower the dO2 with fluid administration or vasoactive agents. Delay or inadequate management of hemodynamics could produce tissue hypoperfusion and hypoxia, affecting surgery outcomes. Knowledge and assessing dO2 during perioperative are essential during anesthesia for high-risk patients. Adequate indices are required to detect and quantify tissue hypoperfusion and the imbalance between DO2 and VO2 during anesthesia. This review presents the mechanism, detection, and quantification of dO2. In addition to interventions to avoid or reduce dO2 and recommendations for anesthesiologists to ensure better results in high-risk surgical patients.

ABSTRACT

Objective:To analyze the relevant factors affecting the frequency and behavior of health check-up among the population of medical examinees. Methods:From January to June 2022, 491 health examinees who had health check-ups at the Second Affiliated Hospital of Shandong First Medical University were included.Basic information of the check-up population and their needs for check-up services (form of check-up, content of check-up, cost of check-up, form of feedback on check-up results, and needs for check-up providers) were obtained based on a questionnaire survey.According to the frequency of previous medical check-up, the examinees were divided into never-check-up group (frequency of check-up was 0, n=45), the irregular check-up group (the interval between two physical examinations >1 year, n=49) and the regular check-up group (frequency of check-up=1 year, n=347). The frequency of previous medical check-up was used as the dependent variable, and the age, gender, education level, income, form of medical check-up and cost of medical check-up were used as independent variables to analyse the relevant factors affecting the frequency of their medical check-ups.The above count data were analyzed by one-way analysis using chi-square test, and the count data with statistically significant differences in one-way analysis were analyzed by binary logistic regression. The statistical software was SPSS 21.0. Results:The study included 491 medical examinees, of which 9.16%(45/491) were in the never-check-up group, 20.16%(99/491) in the irregular check-up group and 70.68%(347/491) in the regular check-up group. The impact of demography characteristics of physical examinees on the frequency of health check-up, and there were statistically significant differences in the frequency of health check-up in terms of gender, job, education, and income ( χ2=21.826, 157.113, 104.764, 45.486, all P<0.05). The understanding of health check-up and the attention paid to one's own health were influencing factors that affected the frequency of health check-up. There were statistically significant differences in the frequency of health check-up in necessity of health check-up, attention to one's own health, health check-up forms, health check-up institutions, health check-up price and health check-up contents ( χ2=9.277, 25.863188.239, 59.115, 39.250, 28.314, all P<0.05). Logistic regression analysis showed gender, job type, monthly income, necessity of health check-up, concerns for items of check-up, forms and institutions of check-up were influencing factor of health check-up frequency(all P<0.05). Conclusion:The health check-up behavior of the population and the frequency of regular medical check-ups are influenced by the factors such as gender, education, occupation, income, form of medical check-up, price of medical check-up, choice of medical check-up institution and the degree of concern for one’s own health. The popularisation of health check-ups for special groups such as men, low-income people, low literacy and insufficient awareness of the importance of one’s own health will help improve the overall health awareness of the population.

ABSTRACT

ABSTRACT Introduction: The interest in spinal endoscopy is rising, particularly among younger spine surgeons. Formalized postgraduate training programs for endoscopic spinal surgery techniques are lacking behind. Methods: The authors performed a retrospective survey study amongst participants of the 2022 AMCICO endoscopic surgery symposium. Descriptive and correlative statistics were done on the surgeon's responses recorded in multiple-choice questions. In addition, surgeons were asked about their clinical experience and preferences with spinal endoscopy, training background, the types of lumbar endoscopic decompression they perform by approach, and future training requirements. SPSS (version 27) statistical software package was used for data analysis. Descriptive statistic measures were used to count responses and calculate the mean, range, standard deviation, and percentages. In addition, chi-square statistics were used to determine the strength of the association between factors. Results: The online survey was accessed by 321 surgeons, of which 92 completed it (53.4%). Demographic data showed the majority of responding surgeons being orthopedic surgeons (73.6%) and under the age of 50 (69.2%), with over half (51.1%) having less than three months of formalized training in endoscopic spinal surgery techniques. Most surgeons practiced uni-portal (58.9%) versus bi-portal (3.4%) spinal endoscopy. The transforaminal approach (65.5%) was preferred over the interlaminar method (34.4%). The bi-portal technique was indicated almost exclusively for the lumbar spine (94.8%). For endoscopically assisted spinal fusions, a uni-portal approach was preferred by 72% of surgeons over a bi-portal procedure (24.5%). 84.1% of respondents were interested in navigation, of which 30.7% preferred optical over electromagnetic technology (18.2%). Robotics was of interest to 51.1% of survey participants. Respondents' bias was estimated with course attendance assessments, with 37% of surgeons having attended all three days, 27.2% two days, and 16.3% one day. One-fifth of responding spine surgeons did not participate in any curriculum activities but completed the survey. The academic impact of the AMCICO endoscopy symposium was high, with 68.1% of respondents indicating interest in continued training and 61.1% of trainees ready to apply their newly acquired knowledge base to clinical practice. Conclusion: The interest in spinal endoscopy surgery techniques and protocols is high among AMCICO members. Many surgeons are interested in learning advanced endoscopic surgical techniques to integrate the technology into their surgical procedure portfolio to address common painful conditions of the degenerative spine beyond herniated discs and foraminal stenosis. The authors concluded that its academic impact was high based on the responses given by the participating surgeons. Level of evidence III; Retrospective study.

ABSTRACT

ABSTRACT BACKGROUND AND OBJECTIVES: Cannabis sativa is a plant that has been used by humankind for many years and is in the media spotlight due to its pharmacological features, being considered the great therapeutic option of the century. With the advent of the Drug Law (Lei de Drogas - Law No. 11,343/2006) there was a starting point for the situation of cannabis in Brazil. Thus, the objective of this narrative review was to discuss information about legal issues regarding cannabis in the Brazilian territory. CONTENTS: The Collegiate Directorate Resolution (Resolução da Diretoria Colegiada - RDC) No. 327 of December 2019, published by Brazilian Health Surveillance Agency (Agência Nacional de Vigilância Sanitária - ANVISA), provides on the procedures for granting health authorization for manufacturing and importation, as well as establishes requirements for marketing, prescription, storage, monitoring and surveillance of cannabis products for medicinal purposes to human use. The Bill of Law (Projeto de Lei - PL) No. 399/2015, proposed to amend article 2 of Law No. 11,343, of August 23, 2006, to enable planting and marketing of drugs containing extracts, substrates or parts of the cannabis plant. CONCLUSION: Cannabis cultivation in Brazil would make a great contribution not only to the pharmaceutical industry, but also to the agricultural industry, generating jobs and reducing raw material costs for drugs. However, the slow pace of Brazilian politics would be an obstacle. There is a need for more consolidated and specific legislation to regulate cannabis.

ABSTRACT

OBJECTIVE In order to analyze the current situation of policy tool allocation in China’s TCM formula granule industry, and provide a theoretical basis for the high-quality development of the industry. METHODS Based on the perspective of policy tools,“ TCM formula granules” was used as the keyword to retrieve the official websites of departments directly under the State Council such as the National Medical Products Administration and the National Administration of Traditional Chinese Medicine, as well as provincial governments. The relevant clauses in the policy documents related to TCM formula granules in China from 1993 to 2022 were encoded, and then the application of policy tools was summarized and classified. RESULTS & CONCLUSIONS A total of 12 national documents and 77 provincial documents were ultimately selected, involving 556 relevant policy clauses. It was found that among the relevant policy tools, environmental policy tools had the highest degree of attention, accounting for 62.6%; the proportions of demand-oriented policy tools and supply-oriented policy tools were less, accounting for 17.8% and 19.6%, respectively. From the perspective of policy tools, the use of demand-oriented policy tools in current policy texts was relatively simple, and various policy tools were given low attention; from the perspective of policy objectives, the proportion of use of environmental, demand-oriented and supply-oriented policy tools were not balanced enough. It is suggested to increase the proportion of demand- oriented policy tools to meet international competition; emphasize the use of supply-oriented policy tools to strictly control product quality; consolidate the use of environmental policy tools to standardize quality standards. On this basis, we will coordinate the overall situation, balance the use of various policy tools and promote the development of TCM formula granule industry.

ABSTRACT

Objective:To understand the research ability, cognition, and training needs of clinical medical staff in a grade A tertiary hospital in Xinjiang and to analyze the influencing factors. Methods:A convenience sampling method was applied to survey the clinical medical staff of our hospital with a questionnaire including general information, a self-assessment scale of research ability, and a survey of research cognition and training needs. A total of 618 questionnaires were collected with 609 valid returned responses, resulting in an effective return rate of 98.54%. Univariate and multiple linear regression analysis were applied to analyze the influencing factors of the total score of clinical medical staff's research ability. Results:The total score of research ability of 609 clinical medical personnel was 60.73±13.59. The results of multiple linear regression showed that participation in scientific research conferences, enthusiasm for scientific research activities, and the need for scientific research training all had positive effects on the self-assessment of scientific research ability, which together explained 52% of the total variance (adjusted R2=0.520, P<0.001). The top three " very important" options for medical staff research training were data analysis, research design, and research topic selection. Conclusions:Medical staff research skills need to be improved and there is a strong need for research training. Managers should refine scientific research management initiatives and provide hierarchical and targeted scientific research training to improve the overall medical staff's scientific research literacy and research ability, thereby promoting the progress of medical care in hospitals.

ABSTRACT

Objective:To survey and analyze the current status and needs of health management for hypertension in empty-nest elderly. Methods:In this cross-sectional study, 677 cases of empty-nest elderly with hypertension who underwent health checkups at the Health Management Center of Sichuan Provincial People′s Hospital from March 1st to June 30th, 2022 were selected consecutively by convenience sampling method to conduct a questionnaire survey. The questionnaires included general demographic information, and the current status and needs of health management for hypertension. A total of 677 questionnaires were distributed, and all the questionnaires were recalled, of which 675 (99.7%) were valid. According to the Classification and Definition of Blood Pressure Levels in the Guidelines for Primary Care of Hypertension (2019) the elderly with mean systolic blood pressure controlled between 90 and 139 mmHg (1 mmHg=0.133 kPa) and mean diastolic blood pressure controlled between 60 and 89 mmHg were set as normal blood pressure group (257 cases), and the ones with mean systolic blood pressure ≥140 mmHg and/or mean diastolic blood pressure ≥90 mmHg were set as abnormal blood pressure group (418 cases). The current management status and needs of hypertension between the two groups was compared, and the χ2 test and multi-factor logistic regression were used to analyze the factors influencing the health management level of hypertension among empty-nest elderly. Results:Most of the empty-nest elderly had their blood pressure monitored once a week (40.30%), most of them had their blood pressure reviewed in outpatient setting for no more than one time per year (40.89%), their blood pressure was mainly controlled by medication (48.30%), more than half of them took medication very regularly (51.84%), and the proportion of the elderly with abnormal blood pressure was much higher than that of those with normal blood pressure (61.93% vs 38.07%). Compared with living with or nearby their children, empty-nest elderly preferred to live alone or with their spouses (63.11% vs 15.85% and 21.04%), and the proportion of those who wanted to be taken care of by their children was much lower than those who wanted to be taken care of by the community and mutual care among the elderly (7.70% vs 58.07% and 34.22%); and the people they wanted to talk to were mainly their children (53.19%). In terms of community services, the needs for medical services in empty-nest elderly was higher than the needs for old-age security and cultural entertainment (43.11% vs 36.15% and 20.74%), and the needs for community medical services was focused on blood pressure measurement (89.04%). The proportions of education level of college and above, high household income, regular medication taking, daily blood pressure monitoring, and desire for frequent child care were significantly lower in the abnormal blood pressure group than those in the normal blood pressure group (34.91% vs 58.35%, 5.17% vs 24.16%, 34.89% vs 63.72%, 28.47% vs 44.75%, and 3.35% vs 10.12%), and the proportion without medical insurance was significantly higher than that in the normotensive group (8.13% vs 3.11%) (all P<0.05). The presence of medical insurance (employee medical insurance, OR=1.986, 95% CI: 1.130-3.492; resident medical insurance, OR=1.291, 95% CI: 1.044-1.598) was positively associated with the health management level of hypertension among empty-nest elderly, while low frequency of blood pressure monitoring (once a week, OR=0.243, 95% CI: 0.101-0.583;≤once a month, OR=0.210, 95% CI: 0.067-0.661) and irregular medication taking (occasionally forget, OR=0.430, 95% CI: 0.186-0.996; often forget, OR=0.361, 95% CI: 0.147-0.886) were negatively associated with the health management level of hypertension among empty-nest elderly (all P<0.05). Conclusions:The health management level of hypertension in empty-nest elderly needs to be improved. Community care, medical services and communication with children are the main needs in the empty-nest elderly. The type of medical insurance, frequency of blood pressure monitoring and medication-taking status are correlated with the hypertension management level in empty-nest elderly.
